Waivers Requests
The two documents below are regulatory waiver requests. Licensed and registered day care providers may choose to include one or more of the waiver requests as part of their health care plan. Waiver requests must be signed by the provider, attached to the health care plan and submitted for approval to the Office of Children and Family Services regional office manager.
