Reviewing the child care law and regulations will help you determine which type of child care you might be interested in pursuing. In addition, reviewing the sample application packages below will assist you in determining the modality of care most appropriate to your location, qualifications, and needs. These are sample applications only; the pages/forms cannot be used to submit any documents to the Office of Children and Family Services.
Please review the sample application prior to requesting an official application package. Any questions, or to request an application be mailed to you, select the appropriate OCFS DCCS Regional Office from the link below. Or, you can contact the Resource and Referral agencies on the attached link for assistance. The application that is mailed to you will include the identification number (facility number) required for processing. The same facility number must be on all documents you submit.
